Full-time and part-time (25 hours or more) IAVI employees are entitled to these benefits, provided they meet the eligibility requirements for each particular plan.
Health & Welfare Plans - effective on your date of hire
- Medical: Open Acces Managed Coice through Aetna; This High Plan is a comprehensive POS Plan which includes in/out of network services and prescription drug benefits; Open Access Exclusive Provider Organization (EO) through Aetna; this Base Plan is only for in-network providers/facilities. Premiums paid by IAVI and employee in both plans.
- Dental: Freedom of Choice Plan through Aetna; comprehensive PPO/DMO which includes in/out of network services and allows you the “freedom” to switch between plans at anytime throughout the plan year; premiums paid by IAVI and employee.
- Vision: Through Vision Service Plan, which offers in/out of network services through an extensive national network of independent eyecare doctors; premiums paid by IAVI and employee.
- Flexible Spending Accounts: Through Benefit Strategies, which allows you to set aside pre-tax dollars to pay for expenses not covered under IAVI benefits plans for health care, dependent care, transit and parking costs with the use of a convenient debit card feature.
Income Protection - effective on your date of hire
- Basic Life Insurance: Basic group life coverage through UNUM equal to two times your annual salary to a maximum of $500,000 without evidence of insurability; premiums paid by IAVI.
- Accidental Death & Dismemberment Insurance: AD&D coverage through UNUM equal to two times your annual salary to a maximum of $500,000 without evidence of insurability; premiums paid by IAVI.
- Voluntary Life Insurance: Employee can elect additional coverage for self in increments of $10,000 up to $500,000; coverage available for spouse and dependent children; premiums paid by employee.
- Short-Term Disability: Payments begin on the eighth consecutive day of illness, and offers benefits up to 6 months of illness; premiums paid by IAVI.
- Long-Term Disability: Provides 60% of monthly earnings up to a monthly maximum of $15,000; premiums paid by IAVI.
403(b) Retirement Plan
- Tax Deferred Annuity: Through TIAA-CREF which allows you to defer pre-tax contributions up to the annual IRS-mandated maximum; effective on your date of hire; company match up to 2% of salary after a six-month waiting period.
- Defined Contribution Plan: Through TIAA-CREF which provides you with contributions made by IAVI based on a percentage of your salary; effective after six months of employment.
Employee Assistance Program
- Program through UNUM to help employees and their family members find solutions for everyday work-life challenges such as child/elder care referrals, legal consultations and financial planning assistance; premiums paid by IAVI.
Tuition Reimbursement Program
- Program to assist eligible employees with the cost of higher education expenses to an annual maximum of $7,000 ($2,000 for employees with IAVI for less than 1 year).
Paid Time-Off
- Vacation Days: Accrued at a rate of 1.25 days per month.
- Sick Days: 10 days per year of employment.
- Personal Days: 3 days per year of employment.
- Holidays: 11 days per year
