IAVI offers benefits to its employees everywhere, but the details below apply only to those in the US
Full-time and part-time (25 hours or more) IAVI employees are entitled to these benefits, provided they meet the eligibility requirements for each particular plan.
Health & Welfare Plans - effective on your date of hire
Medical: CIGNA (Open Access Plus) Plan which includes in/out of network services and prescription drug benefit; which includes in/out of network services and prescription drug benefits.
Dental: Flexible Dual Choice Plan through CIGNA Dental; comprehensive PPO/DMO which includes in/out of network services and allows you the “freedom” to switch between plans at anytime throughout the plan year.
Vision: through Vision Service Plan which offers in/out of network services through an extensive national network of independent eye care doctors.
Flexible Spending Accounts: through Benefit Strategies which offers health and dependent care as well as transit and parking accounts.
Income Protection - effective on your date of hire
Life Insurance: coverage through Lincoln Financial equal to two times your annual salary to a maximum of $500,000 without evidence of insurability.
Accidental Death & Dismemberment Insurance: AD&D coverage through Lincoln Financial equal to two times your annual salary to a maximum of $500,000 without evidence of insurability.
Voluntary Life Insurance & Voluntary AD&D: employee can elect additional coverage through Lincoln Financial for self in increments of $10,000 up to $500,000; coverage available for spouse and dependent children.
Short-Term Disability: payments begin on the eighth consecutive day of illness, and offers benefits up to 6 months of illness.
Long-Term Disability: provides 60% of monthly earnings up to a monthly maximum of $15,000.
403(b) Retirement Plan
Tax Deferred Annuity: through Transamerica which allows you to defer pre-tax contributions up to the annual IRS-mandated maximum; effective on your date of hire; company match up to 2% of salary after a six-month waiting period.
Defined Contribution Plan: through Transamerica which provides you with contributions made by IAVI based on a percentage of your salary; effective after six months of employment. IAVI will contribute a percentage of your salary into a 403(b) retirement plan on your behalf, regardless of whether you make any voluntary contributions to the plan.
You will be 100% vested in IAVI’s Company Contributions and Company Match (including any earnings on such amounts) after accruing three (3) years of vesting service under the Plan. You will be credited with one (1) year of vesting service in this plan for each year you work at least 1,000 hours per year. You are always 100% vested in your own contributions and the earnings on those contributions.
Employee Assistance Program
Program through Lincoln Financial to help employees and their family members find solutions for everyday work-life challenges such as child/elder care referrals, legal consultations and financial planning assistance.
Tuition Reimbursement Program
Program to assist eligible employees with the cost of higher education expenses to an annual maximum of $7,000. ($2,000 for employees with IAVI for less than 1 year).
Paid Time Off
Vacation Days: Accrued at a rate of 1.25 days per month
Sick Days: 10 days per year of employment
Personal Days: 3 days per year of employment
Holidays: 11 days per year
